HSA Safe Environment
Parents and Volunteers,
Your dedication to our Saint Agnes students and our school is invaluable and we could not operate without your help. To provide our children with the safest environment the Archdiocese has required that all volunteers complete the following 3 requirements.. The generosity of your time in completing these items is greatly appreciated. Listed below are the requirements, which can be accessed at archphila.org/protection/index.htm, then click on safe environment .
*Keep a copy of all 3 certificates for your own records
1. Complete a Pennsylvania State Police Request for Criminal Record Check: You can do this online at https://epatch.state.pa.us/RecordCheckEntry.jsp. You should receive your certificate instantaneously, print a copy or save to your computer. You will need two copies of the certificate. One copy should be forwarded to the rectory c/o Martine Simonis or email to her martinesimonis@yahoo.com , and the second copy is needed for the Child Abuse History Clearance. This is valid for 5 years.
2. Complete the Pennsylvania Department of Public Welfare Child Abuse History Clearance: Access this form and the instructions at : http://www.dpw.state.pa.us/ucmprd/groups/webcontent/documents/form/s_001762.pdf
. Fill out the Pennsylvania Child Abuse History Clearance Form completely. In the box titled “Purpose of Clearance” check VOLUNTEER. The form requires that you list EVERYONE you have lived with, and everywhere you have lived since 1975. Do not forget to include your parents if you lived with them after 1975 (forms have been sent back for not including the complete information). You will receive your child abuse clearance from Harrisburg usually within 3 weeks. Please take a copy of the Pa. State Criminal Record Check Certificate and the Pennsylvania Child Abuse Clearance Certificate c/o Martine Simonis at the rectory. Please keep a copy of these documents, they are valid for 5 years.
3. Attend a Safe Environment Class: The schedule for the Safe Environment classes can be found on the archdiocesan website: . You may enroll for a class at www.virtus.org. . Please return a copy of the class certification to the rectory c/o Martine Simonis. This is required only once.
